The type of mongolism described in the Milledgeville mongoloid is due to an independent chromosome 21, plus a 2\q, 21< 7 zygosome produced by uniting the long arms of a quasi complete 21 with the long arms of a 21 that has lost its centromere but retained its satellites. This zygosome belongs to a rare group of 2\q, 2\q reciprocal translocation structures in which various amounts of the two chromosomes 21 join in either the nonsatellited sequence of p ab ab or in the p ab ba order present in the Milledgeville case. In the p ab ba type, biterminal satellites may be observed in some chromosome spreads of some patients. In other supposed p ab ba patients, satellites have never been seen. Aside from the Milledgeville case, a number of authors have independently reported a case of a tandemly arranged 2lq/2\q translocation that is bisatellited. The varying amounts of the q segments present in the zygosome may account in large measure for the physical and mental deviation of these mongoloid patients from the usual spectrum of characteristics typical of mongoloids bearing three independent chromosomes 21.
