Sir, We were interested in the recent article published in Brain proposing the new clinical, MRI and neuropathology diagnostic criteria of chronic lymphocytic inflammation with pontine perivascular enhancement responsive to steroids (CLIPPERS) by Tobin and colleagues (2017). Here, we report two recent cases fulfilling the clinical and MRI criteria of CLIPPERS, but which failed to meet the new neuropathological criteria by presenting increased number of perivascular CD20-positive B lymphocytes. As there were no specific neuropathological biomarkers for CLIPPERS, and no other better explanation for neuropathological presentation of these two cases, we suggest that we should not exclude non-CLIPPERS simply because they did not meet the neuropathological criteria outlined by the authors, and the spectrum needs to be broadened.
