INSULIN-like growth factor I (IGF-I), the most important somatomedin, is produced in the liver and other tissues following stimulation by GH. Serum IGF-I levels are therefore dependent on GH secretion. GH hypersecretion induces high IGF-I levels and GH deficiency causes low IGF-I levels. However, low IGF-I levels in spite of normal or increased GH secretion are observed in such pathological conditions as protein-calorie malnutrition [1], liver diseases, uncontrolled diabetes mellitus [2] and Laron dwarfism [3]. In this paper, we present a case of low IGF-I production with GH hypersecretion which occurred in a patient with rheumatoid arthritis (RA) and diabetes mellitus (DM). Although the causes of low IGF-I production were unknown, we investigated whether GH receptor unresponsiveness was present in the
