T-ALL is a rare type of aggressive leukemia characterized by activating mutations in the signaling protein NOTCH1 (ref. 5). Treatment of this disease with conventional chemotherapy is frequently associated with therapy failure, and the prognosis of patients with relapsed or refractory T-ALL is extremely poor. Several Notch inhibitors, known as GSIs, have been developed. Although these compounds have considerable preclinical activity, their use in clinical trials has not yet translated into significant human clinical responses, in part owing to the development of resistance6.
