A 44-year-old Japanese male without significant past history presented with swelling of the left parotid gland. Fine-needle aspiration cytological examination was performed. Conventionally-stained Papanicolaou smear revealed abundant epithelial cell clusters in a myxoid background. Most clusters were cohesive; however, dissociated cells in a myxoid background were observed. These epithelial cells exhibited bland, round-to-oval nuclei without nucleoli. Myxoid material showed metachromasia by Giemsa staining. These cytological features were typical for pleomorphic adenoma (PA). A notable finding was the presence of round-to-oval structures within the epithelial clusters (Fig. 1A, 1B). These structures comprised radially arranged needle-shaped crystalloids (Fig. 1A, 1B). Histopathological examination of the resected left parotid gland tumor demonstrated that the tumor was well-circumscribed by a fibrous capsule; furthermore, proliferation of epithelial and neoplastic myoepithelial cells was observed within a myxochondroid material. Tubular formation was observed. Both epithelial and neoplastic myoepithelial cells had bland, round-to-oval
