Alfandre and Geppert correctly highlighted the importance of recognizing the issue of sexual harassment by patients and its effect on professionalism and the patient-physician therapeutic relationship. 1 However, their article may prematurely focus on potential solutions before completely characterizing the problem of sexual harassment of physicians by patients.The example of harassment provided in the article highlights a comment that was directed at a physician’s appearance, where use of a script could potentially be helpful. However, it is important to consider sexual harassment from patients can, at times, be much more severe than the example included and may entail comments about a physician’s specific body parts, lewd behaviors, or even unwanted physical contact. All of these patient actions have the potential for severe and ongoing emotional consequences for affected physicians, and the provision of a standard script would not address these issues. The reliance on scripts puts the onus on the part of the physician to address the behavior when he or she may not be in a position to do so. Furthermore, rather than trying to identify and address the actions of a single wrong actor, we need to create a system that increases safety at multiple levels.
