MethodsIn a prospective observational study, blind ultrasound sweeps were undertaken. Manual assessment was performed by one reader for 109 subjects (81 in second trimester, 28 in third trimester). Six anatomical structures were manually labelled across all sweeps: midline falx, cavum septi pellucidi, lateral ventricles, choroid plexus, cerebellum, and cisterna magna. The count and percentage of visibility of each structure were computed across all sweeps.ResultsAll assessed brain structures were visible in the blind sweeps at least 45% of the time. This was higher in the second trimester (> 55%). The most seen structure was the midline falx (91/109 (83%) subjects;(75/81 (93%) in the second trimester), followed by choroid plexus in (76/109 (70%) subjects (72/81 (89%) in the second trimester). Cavum septi pellucidi, lateral ventricles, cerebellum and cisterna magna were visible in 46-53% of cases (57-61% in the second trimester). The complete set of all structures were visualised in 27 (25%) subjects; 25 (31%) in the second and only 2 (7%) in the third trimester.ConclusionsFetal anatomical brain structures are visible in blind sweeps in a significant proportion of cases, but this depends on the individual structure and visualisation is higher in the second than the third trimester. Future work should assess agreement with additional readers and clinical methods for optimising structure visibility to allow assessment of fetal brain anatomy from blind sweeps.
