At 6: 00am each morning, John parks the “methadone van” in a large community center parking lot in Woonsocket, Rhode Island. In the county with the highest opioid overdose fatality rate in the state, the CODAC Behavioral Healthcare Mobile Health Unit brings treatment to a hot spot of the rising opioid overdose epidemic. In 2007, the Drug Enforcement Agency (DEA) ceased approving these “methadone vans.” Now, after new DEA rules, this clinic-on-wheels starts patients on life-saving treatment, driving directly to the communities most in need.A trickle of patients get dosed in an admittedly cramped space. They receive a warm greeting from a peer coach, a nurse, a social worker, John, and twice a week, an addiction physician. Despite the narrow area, patients often stay a moment to indulge in the low-threshold ethos of the van culture. One of my first van patients, Josh, explained,“I used to have to drive 20 minutes to the clinic every day before work, now I just walk here. It’sa lifesaver.”
