DiscussionSyringomyelia has a variable presentation which can include:(1) asymmetric weakness and atrophy of the hands and arms,(2) upper and lower limb spasticity,(3) areflexia of the upper extremities,(4) dissociated sensory loss in the neck and arms (classic cape-like distribution), and (5) increased tone and hyperreflexia of the lower extremities. 1, 2 Symptoms of syringomyelia tend to be multisegmental with upper motor neuron lower extremity paresis 3 and lower motor neuron upper extremity weakness. 4 The current case appears to be the first one, in the absence of scoliosis and a tethered cord, to show a rapidly progressive and isolated lower extremity weakness as the sole presenting symptom of an extensive syringomyelia.
