Secondary prevention of coronary heart disease (CHD) is a term used to describe therapeutic efforts in patients with established disease. For patients who have sustained a myocardial infarction or who have developed clinical manifestations of coronary atherosclerosis, strategies for secondary prevention remain a subject of much controversy as well as burgeoning interest. Treatment of coronary disease has received so much attention because of its prevalence. Coronary disease affects 1 1/2 million Americans annually and is the leading cause of death in the United States and many countries in Western Europe. Secondary prevention is controversial, in part, because the factors influencing the prognosis of patients with coronary disease are not fully understood, and the rationale for various interventions is not always evident.
