The implementation of alternative blood conservation strategies to their full extent before preoperative autologous blood donation (PABD) is applied might reduce costs and blood donation risks for patients undergoing elective surgery. If PABD has to be performed, it has to be arranged as effective as possible. This includes an optimal time schedule which can be achieved by the preparation of autologous red blood cell (RBC) concentrates or whole blood with an optimal shelf life. Preoperative active immunization against hepatitis A and B should be considered, too. Early screening for iron deficiency including measurement of ferritin and hematocrit values in all patients who have to undergo elective surgery, including those who are not eligible for PABD, should be performed. In the planning of blood conservation strategies, special attention should be paid to patients with low blood volume and low initial hematocrit.
