The authors also go to some length to explain the notable lack of patients in their series with distal hypospadias. True, distal forms should not impair fertility, but effects on voiding and cosmetic appearance are not so easily discounted. Certainly boys with proximal glanular hypospadias might have a deflected and/or spraying urinary stream, warranting correction. Similarly, the hooded prepuce that appears neither circumcised nor natural might call attention to the penile abnormality even if meatal displacement is less obvious. Given excellent functional and cosmetic outcomes from currently used techniques to correct distal hypospadias, and the uncertainty in determining which infants might have future concerns if left uncorrected, it is surprising that the authors appear to advocate a rather nihilistic attitude towards its repair.
