Materials and methods. Serum amyloid A protein was measured in a competitive binding assay using'251-amyloid A fibril protein for binding to the wells of flexible microtitration plates coated with antiamyloid A antibodies (4). Aliquots of test serum were heated at 60 C for 1 hour prior to the addition of the radioimmunoassay buffer. SAA was measured in triplicate wells and expressed in terms of amyloid A equivalents (pg/ml). With this assay, normal values of SSA are< I pg/ml.C-reactive protein was measured by rate nephelometry using the Beckman kit for CRP and the Beckman ICS Analyzer 11 nephelometer (Beckman Instruments, Brea, CA). Normal values are those values that are less than the threshold concentration for the method, which is 6 pg/ml. Results. Prior to treatment there were moderate elevations of SAA and CRP, along with a high ESR and thrombocytosis (Figure 1). Intravenous penicillin treatment was begun at 9: OO AM on December 1, 1983, day 0. Approximately 12 hours later, the patient developed a frontal headache and had a shaking chill. His temperature rose to 39.6" C, and his blood pressure dropped to 82/48 mm Hg. Penicillin was discontinued. Blood and urine cultures obtained during the febrile episode were negative.
