RIMARY HEALTH care research and practice are in an important period of transition with respect to the roles that communities play in the health of people. Both practitioners and educatorsalike are moving from defining “community” as a physical setting in which care takes place toward an understanding thata community is a “living” organism with interactive webs oftiesamong organizations, neighborhoods, families, and friends. As such, the health of this organism is understood to be directly related to the health of the people who are its members. An accompanying understanding is that important determinants of health-related behavior change are embedded in the relationships that tie individuals to organizations, neighborhoods, families, and friendsThe views expressed in this publication are solely those of the authors and not necessarily those of the Department of Health and Human Services. The term primary health care is used by the authors to encompass the variety of definitions of primary care and primary health care found in the literature during the past 20 years.
