The circadian rhythm of plasma corticoid and IOP [Intra-ocular pressure] levels are positively correlated in glaucoma, the best correlation occurring when IOP levels are paired with corticoid levels which had occurred 4 hours previously. Interruption of the rising phase of the plasma corticoids by a beta-hydroxylase inhibitor is followed by interruption of the rising phase of IOP, again with a 4-hour lag. These findings suggest that the circadian fluctuations of IOP in glaucoma are related to and perhaps determined by the circadian fluctuations of plasma corticoids. The circadian rhythm of aqueous outflow facility is inversely related to IOP in both normal and glaucomatous eyes. These findings suggest that IOP may be the main factor controlling the circadian rhythm of outflow facility.
