MethodsThis cohort study was approved by the University of Utah institutional review board with a waiver for consent because the study posed minimal risk and consent was not feasible to obtain due to the retrospective design. Strengthening the Reporting of Observational Studies in Epidemiology (STROBE) reporting guidelines were followed.
