SCALATING health care costs, which may increase sixfold ($322 billionto $1.9 trillion) between 1984 and2000 (Tyson 8 Merrill, 1984), pose a crisis for Americans. One cost-contain-ment strategy, prospective payment systems (PPSs), was implemented by the federal govern-ment to promote efficient utilization of inpatient resources. Although originally targeting only Medicare patients, the promulgation of PPS to other third-party payers has broadened its im-pact to virtually all patients.
