Success in pediatric transplantation is guided by multiple factors, both scientific and social in nature. The wide range of age groups encompassed under the term pediatric provides health care professionals with a variety of patients ranging from post-partum to early adulthood. This patient population has inherent growth and developmental stages. As such, outcomes are affected by both pathophysiological entities as well as by psychosocial events associated with the various life stages. With this mindset, we read with great interest and pleasure Drs Rumbo, Shneider, and Emre’s manuscript on Utility of azathioprine metabolite measurements in post-transplant recurrent autoimmune and immune-mediated hepatitis (1). In it, they outline a disease with a still unclear etiology that occurs at various age groups. Its treatment is by means of an otherwise seldom-used agent in transplantation. This pharmaceutical product is found to have ageassociated variations in dosing. To make the plot even more intriguing, the outcome not only depends on the adequacy of treatment but also on the compliance of patients and their caregivers.
