Six patients with dermatitis herpetiformis (DH) and “flat” proximal intestinal lesions were placed on strict gluten-free diets for 2 1/2 to 6 months. Multiple biopsies were obtained from the whole length of the duodenum and up to the first two loops of jejunum, before and after gluten withdrawal. A striking improvement in villous architecture occurred in all 6 patients. Three of the 6 patients had definite steatorrhea which improved after gluten withdrawal. It is concluded that these 6 patients with DH and flat proximal intestinal mucosa have celiac sprue.
