SPREAD OF SEIZURE DISCHARGES IN EPILEPSY - ANATOMICAL AND ELECTROPHYSIOLOGICAL CONSIDERATIONS

In an attempt to trace the spread of spontaneous electrical seizure activity within the brain of epileptics, the opportunity has been taken to follow the occurrence and temporal development of these outbursts in patients with epilepsy who have had indwelling electrodes placed for purposes of diagnosis, lateralization and later therapeutic surgery. A computer program has been used to detect the essential frequency or frequencies in each unprovoked burst of abnormal wavetrains and to follow their passage from the initiating driving zone to the other parts of the brain that are reached by this spread. The path followed by spontaneous seizure activity in spreading to other structures is a feature peculiar to each individual and can be established for that patient by this method. The possibility offered for a type of therapeutic surgery more restricted than temporal lobectomy is mentioned.
