Nine patients with severe long-standing alopecia areata (AA) including 5 patients with at least 0% patchy scalp hair loss and 4 patients with alopecia totalis or universalis, were treated with repeated pulses of dexamethasone (100 mg i.v. daily for 3 days every 4 weeks). The patients were 16-56 years of age (mean: 36 years). Eight were female and one was male. The total duration of the disease prior to the dexamethasone pulse therapy averaged 14 years (14 months to 41 years). The patients received 4 to 10 cycles (mean: 6 cycles) of dexamethasone pulses. During the treatment only one patient showed a good, but temporary response (30% regrowth after 6 months). After the end of the treatment 2 patients developed a maximum regrowth of less than 5%, while no improvement or further hair loss was observed in the other 6 patients. In conclusion, treatment of patients suffering from long-standing, severe forms of with repeated pulses of dexamethasone gave unsatisfactory results.
