A great deal of experimental and clinical evidence has indicated that the kidney not only has the potential for increasing arterial pressure through the renin-angiotensin 11-aldosterone system but also has a blood pressure-lowн ering function. This was evident in the initial experiн ments of Goldblatt et al.[1] when they found only tranн sient and unpredictable hypertension following renal arн tery'constriction when the contralateral kidney was left intact. In contrast, when the contralateral kidney was removed, a severe and sustained hypertension would invariably occur. The anti-hypertensive effect of the kidн ney had been raised by the experiments of Chanutin and Ferris [2] 2 years previous to Goldblatt's experiment when they demonstrated that a reduction in renal mass caused hypertension. Subsequent experiments in the rat [3] and dog [4] in the late 1940s demonstrated that folн lowing bilateral nephrectomy blood pressure rose and although the hypertension was worse when salt and water was administered, volume expansion was not necessary for the hypertensive response. The experiments of Grollman et al.[4] in which blood pressure was compared in dogs with ureters diverted into the inferior vena cava and in animals having a bilateral nephrectomy demonstrated that the presence of the kidneys lessened the hypertension in uremia. Since volume expansion was equal in both groups of animals, the concept of renoprival hypertension became recognized.
