Mammography, breast self-examination (BSE), and clinical breast examination (CBE) are the commonly employed breast-cancer screening modalities. Of these, screening CBE has received the least attention in medical literature. No randomized trials have yet reported the impact of screening by CBE alone on breast cancer mortality, although one trial is underway in India. Four of the eight screening mammography trials have also included CBE as a screening modality, however, and results indicate that the sensitivity of two-modality screening (mammography and CBE) is greater than that of mammography alone, because screening CBE can detect cancers missed by mammography. Thus, CBE should remain an integral part of all screening mammography programs. In some countries, mammography screening is regarded as too expensive, and health policy-makers are considering implementing breast cancer screening programs based on CBE alone.
