The conservative management of watching with screening of small acoustic neuromas reserving surgery for those that enlarge is now a well recognised practice that is superior to initial surgical intervention in preserving the hearing and facial nerve function (1). What has not been reported before is that watching with screening is also cheaper, especially as the cost of MRI scanning is likely to continue to fall in comparison to that of surgery. So though the paper reported on page 438 is based on Canadian costs, it is likely to be mirrored elsewhere in the developed world. The difference in cost is material with conservative management being at least half the cost of surgery for everyone. From the Canadian figures it is only when the surgery rate in those being conservatively managed initially reaches 80% do the costs equate.
