Apparent diffusion corfficient (ADC) imaging is a sensitive method to detect brain edema. We examined the ultrasmctural localization of edema fluid and the correlation of regional tissue water content to ADC \ d u e in cytotoxic type of and vasogenic type of brain edema. Merhodr. Adult csu were subjected to either middle cerebral artery occlusion or cortical cold lesion under anesthesia and monitohng of physiologic parameters. an0 ADC changes, blood-brain barrier (BBB) changes, regional tissue water conrent and histolou were examined. Results. In cytotoxic edema at early ischemia. ADC decreased in parallel to the incrrase of water content in both gray and white matter. The correlation lines showed different slopes and intercepts. The decrease was ccrrelated with the swelling of astrocytes and oligodcndroglia. In vasogenic edema after cold lesion. ADC increased in parallel to the increase of tissue water content. Enlargement of inter-fiber space of the white matter was correlated with the increase of ADC. Comments. ADC is a powerful tool for the research of brain edema pathophysiology. The observed different localization of edema fluid may explain the different behavior of ADC among different types of edema.
