An atrial septostomy was performed by the balloon catheter technique in a 21 month old child who had tricuspid atresia with a small foramen ovale and severe congestive heart failure after two systemic artery to pulmonary artery shunts. This technique has previously been reported in infancy but not in a child of this age.An immediate decrease in the mean pressure between the atria was demonstrated, and further improvement in the hemodynamic status was evidenced on another catheterization 2 weeks later. Changes in left ventricular function were also noted. This improvement in function was noted on follow-up examination 21 months later.
