Under the direction of Professor Mitsuo Homma, the IVth International Sjogren’s Syndrome Meeting was held August 11-13, 1993 in Tokyo. Clinical sessions were devoted to classification and treatment of dry eyes and dry mouth; basic research sessions focused on the failure of tolerance mechanisms, genetic and environmental cofactors, and cytokine and neuroendocrine mechanisms. The recently published European Cooperative Study Group preliminary criteria for the classification of Sjogren’s syndrome (1) generated extensive discussion. The key point of debate was that these criteria could be fulfilled in the absence of either autoantibodies or positive findings on labial salivary gland biopsy. Several groups of investigators expressed concern that the criteria were too vague and that the group of patients classified as having Sjogren’s syndrome would be too heterogeneous to serve as a basis for epidemiologic or clinical studies. A surprising aspect of clinical studies was that symptoms of oral dryness correlated poorly with volume of saliva, suggesting a role for specific proteins and mucins in the maintainence of mucosal surface integrity and in symptoms of discomfort. Biochemical studies are underway to characterize the specific mucins (glycoproteins), adhesive molecules (integrins, selectins, cadherins), and low molecular weight proteins (defensins, histatins) that are responsible for maintaining the differentiation of the corneal/conjunc-
