Foo, Roshidah and Aimy report reduced whole blood platelet aggregation in a carefully controlled study of habitual smokers (1). They suggest that their results preclude a role for enhanced platelet activity in ischaemic heart disease in subjects who smoke. Cross sectional results from the Caerphilly Heart Disease Study also show a markedly lower sensitivity of platelets to adenosine diphosphate in whole blood among habitual smokers who had abstained for over an hour (2) (Table 1). However, the sensitivity to adenosine diphosphate was found to be increased in those subjects who had smoked cigarettes shortly before venesection. This observation is consistent with experimental studies of the acute responses of platelets to smoking (3, 4, 5).
