Therefore, that measurable effects on disease activity indices (which are designed to capture symptoms of inflammation among people in remission) were not observed is not surprising. Use of alternative tools to capture non-inflammatory symptoms and assess overlapping disorders of gut–brain interaction could improve how disease activity and symptom control in people with quiescent IBD are conceptualised. Meaningful advancements in psychological therapies for IBD are under way. These findings suggest that newer third-wave therapies11 that place greater emphasis on the processes of change than traditional CBT could be particularly useful. Future refinements in approaches to delivering psychological therapies and selecting people with IBD for psychological treatment will enhance individualised care for these people. Riggott and colleagues offer an updated understanding of psychological therapy in people with IBD while highlighting major knowledge gaps, including the lack of prospective trials of people with active IBD or over lapping IBD and IBS and the lack of research examining differences between Crohn’s disease and ulcerative colitis. Despite these limitations, biopsychosocial perspectives should be embraced and the ways that the health and wellness of a person are defined when delivering IBD care should be reconsidered.
