The development of predictors of susceptibility to drug-induced memory deficits in the elderly would be valuable because centrally acting medications, such as benzodiazepines, are widely prescribed in this population, and older individuals may be more sensitive to some of the adverse central effects.We demonstrated that an acute low (2.5 mg) dosage of diazepam resulted in memory deficits in the elderly but not in young, healthy volunteers.[1] Interestingly, the elderly had significantly lower plasma diazepam concentrations than the young. Possibly, pharmacodynamic mechanisms (perhaps an increased intrinsic sensitivity to the drug), along with well-established age-related pharmacokinetic changes [2]-decreased clearance resulting in increased plasma drug accumulation-may also contribute to the increased sensitivity of the elderly to benzodiazepine-induced cognitive toxicity. Although our ability to investigate in vivo complex pharmacodynamic mechanisms that may accompany aging is limited, we can examine genetic markers that may predict central nervous system vulnerability to adverse drug effects.
