PREvoUs CoNNIUNICATIoNs*** indicate that laparotomy just prior to thoracotomy for bronchogenic carcinoma is a useful adjunctive proccdure for the discovery of occult extrapulmonary metastases. The basis for this exploration is the high incidence of metastases from primary bronchial cancer to the organs and lymph nodes of the upper abdomen. Determination of such metastases with the expectation of a lim-ited survival time would suggest the futil-ity of a curative pulmonary resection. On the other hand, when extrathoracic spread is not apparent, a standard or extended pul-monary resection may well offer the prom-ise of prolonged survival. Prethoracotomy abdominal exploration is a means of selecting those patients who will conceivably benefit from pulmonary resection and avoiding thoracotomy in those who will not. This report describes the results of laparotomy in 100 consecutive patients who were clinically suspected of having carcinoma of the lung and were candidates for thoracotomy.
