DISSECTING aneurysms of the proximal aorta often cause sudden impairment of cerebral blood flow and thus present as a neurologic illness of apoplectic onset. Signs of disordered cerebral function commonly dominate the clinical picture in such cases and thereby tend to obscure recognition of the primary lesion. Although many clinical reports indicate the frequency with which the brain may be affected in aortic dissection, in only 12 such cases has there been a complete necropsy examination of the brain and major branches of the aortic arch.Since recently developed surgical techniques have substantially improved the prognosis of aortic dissection, the necessity for prompt and accurate diagnosis has assumed great importance. This report presents the clinical and pathological features of 16 cases of dissecting aortic aneurysm in which related cerebral dysfunction occurred. Two syndromes emerge from consideration of these cases and these, if recognized, should facilitate the prompt diagnosis of this condition and hence its effective management.
