The critical closing pressure (CCP) of the vessels was estimated as the difference between the digital systolic blood pressure and the air pressure required on the finger to just prevent flow of blood cells in the capillary loops. Systolic pressure was estimated by an auscultatory method and presence or absence of flow in the loops was detected by direct microscopic examination. The method is valid and does not simply measure a local intravascular pressure or some rheologic factor. In 6 subjects the mean CCP was 40 and 13 mm Hg when they were heated and cooled respectively. Anesthesia of the digital nerves reduced the CCP by about 50% in 3 subjects. The mean CCP for a group of 10 patients with essential hypertension was 52 and 19 mm Hg during regulated vasoconstriction and vasodilatation respectively compared to the corresponding means of 28 and. 12 mm Hg for a group of 6 normal subjects of comparable age. The greater CCP in the hypertensives indicates that an unusually high constrictor tone in the smaller vessels is at least partially responsible for the abnormally high peripheral resistance in these patients.
