IMPROVEMENTS in radiopaque contrast media, since the introduction of cerebral arteriography,’-z now permit positive arteriographic confirmation of cerebrovascular disease. For many years carotid arteriography has permitted study of most of the hemispheric intracranial vessels, but recent technical advances have made it possible to investigate the intracranial distribution of the vertebral-basilar arterial system as well as the extracranial course of the cerebral arteries from the aortic arch to the brain.3-10 Careful clinical study, including history, general medical and neurologic examination with auscultation of the vessels in the neck,11 make accurate diagnosis possible in the majority of patients suffering from occlusive cerebrovascular disease. While electroencephalography, carotid compression tests, ophthalmodynamometry, and arm-toretina circulation times12.13 aid in establishing the diagnosis, arteriography, at present, remains the only method for providing positive confirmation. Arteriographic investigation of large series of patients with symptoms of cerebral ischemia has indicated that surgically accessible lesions of extracranial vessels are common. The purpose of this cammunication is to present the findings in a systematic arteriographic study designed to define the incidence of surgically accessible lesions in cases of cerebral ischemia admitted to a large general hospital.
