SNOMED CT is a comprehensive multilingual clinical terminology used in electronic health records and interoperability. It is a merger of two earlier systems, SNOMED RT and the Read Codes. Its components are concepts (codes), descriptions (terms) and relationships. Concepts are organized into hierarchies. SNOMED CT expressions are either a single concept (pre-coordinated) or more complex post-coordinated expressions using compositional grammar. Reference sets are used to provide subsets of the whole terminology for specific purposes. The SNOMED CT Concept model is the set of rules that governs how concepts are permitted to be modeled using relationships to other concepts. Certain attributes and values can be applied to each type of concept.
