The writers of this letter have examined, over a period of several years, hair samples from more than 2500 children from the Caribbean and Central and South America who showed different degrees of protein-calorie malnutrition (PCM), and here they set down some general observations.Hypochromotricia (abnormal and reduced hair pigmentation) is a variable sign; it is not present in many severe cases of kwashiorkor. If present in kwashiorkor it is obvious, but it is less frequent and less obvious in marasmus.It is not useful as a diagnostic guide to a child's present condition, since it is an indication of some nutritional insult which occurred 1-3 months before showing its effect in the hair. It is useful for community surveys if its limitations are understood, and it is a sign which is easily visible and can be recorded by paramedical personnel. The different shades of colouration have little predictive value in kwashiorkor in relation to severity or chronicity. The hair shaft provides a readily available nutritional history since the duration of a period of malnutrition may be calculated from the length of the area of dyspigmentation (hair grows at a rate of 0.2 mm per day). Repeated bands of lightened hair colour suggest distinct periods of malnutrition. Local conditions or habits which may affect the picture should be borne in mind,e.g. weathering from sun and salt air, dry dusty conditions, or the use of hair dyes or oils.
