The development of selective coronary arteriography in 1958 using a flexible catheter introduced via brachial arteriotomy [I] was followed by a percutaneous femoral approach using preformed catheters [2, 3]. The original designs of the Judkins catheters have shown remarkable longevity and remain predominant today. Guiding catheters for percutaneous transluminal coronary angioplasty (PTCA) adopted the Judkins curves, but with slightly shorter distal tips and stiffer shafts than their diagnostic counterparts. Cannulating a coronary artery for PTCA requires that the guiding catheters are engaged in a stable and coaxial position for support (“back-up”) to permit the dilatation catheter to be advanced through a tortuous artery and across a subtotal stenosis or total occlusion. Careful guiding catheter selection is necessary for passage of ever smaller and more sophisticated dilatation devices (eg, Probe [United States Catheter and Instrument Co., a division of CR Bard, Billerica, MA]) into the target arteries and across severe stenosesFor the left coronary artery (LCA), the left Judkins guiding catheter is usually satisfactory; occasionally, for more posterior or superior left main origins, other guiding catheters, eg, Amplatz, are required. However, the greater variability of the origin and proximal segment of the right coronary artery (RCA) frequently poses a problem for coaxial guiding catheter cannulation. In some cases, the selection and usage of the appropriate guiding catheter may constitute the major challenge for right coronary angioplasty.
