Differentiating retinitis pigmentosa from certain types of bilateral chorioretinitis is not always easy. Moreover, fundus changes similar to those seen in retinitis pigmentosa may also develop after injury to the retina. This report does not intend to go into al1 the details of differential diagnosis; attention is called, however, to the symmetrical development of pigmentation which is a highly important feature in the diagnosis. pigmentosa. It has been found in many cases that pigmentation, which is commonly considered simply a sequel to the destruction of the neuro-epithelium, does not always begin circularly. Frequently, it makes its first appearance at corresponding circumscribed sites of the two eyegrounds.Except in the very few cases of unilateral involvement the disease usually affects the eyegrounds simultaneously, with equal intensity and, significantly, the pigmentation tends to appear+ at identical sites on the fundus. Unfortunately, the patients usually seek advice only when the clinical and fundus changes of retinitis pigmentosa are already marked. In such cases, of course, neither early diagnosis nor early symptoms can be described. As a matter of fact, it is almost exclusively in children of parents known to have retinitis pigmentosa as a familial disease (usually dominant inheritante) that the early stages and early changes of the disease can be studied.
