The lesion classically associated with the inhalation of inorganic mineral dusts is interstitial fibrosis, either diffuse, for example in asbestosis, or nodular, for example in simple silicosis. Similarly, the classic functional change associated with mineral dust exposure is a restrictive deficit. However, recent morphologic studies in man as well as experimental studies in animals have shown that mineral dusts also produce lesions in the small airways. At the same time, there has been a growing recognition that mineral dust exposure in some instances is associated with airflow obstruction rather than restriction. In this article I shall briefly review the morphologic findings seen in the airways in dustexposed humans and animals, consider the possible pathogenesis of these lesions, and discuss where such changes may fit into the concept of dust-induced airflow obstruction.
