We thank the Chiun-Feng Tsai and colleagues for their interest in our Delphi study on the classification of fibrotic disease 1 and are grateful for the opportunity to respond. We note the concern of response bias. While we have reported on both the geographical region and speciality of the first round responders, we were unable to collect this information from subsequent survey rounds. The survey was shared and signposted to experts internationally, and consensus drawn predominantly from respondents who were largely situated in Europe and the Americas. We are aware that these responses may differ from those from other parts of the globe and accept that as a limitation of this work. While we tried to disseminate the survey as widely as possible, we were ultimately limited to including those who responded. The number of responses were more limited in later rounds, which was consistent across all disease specialties (Table 1). Nervous and reproductive systems were not sufficiently represented in subsequent rounds for consensus definition, contributing instead to clinical agreement. We are aware that respiratory, eg had a good response rate throughout, but others dropped off. Again, we recognize this as a limitation but something we had little control over.
