THE EFFECT OF GLUCOSE LOADS ON RENAL URIC ACID EXCRETION IN DIABETIC PATIENTS.

Despite the occurrence of hyperuricemia in diabetic patients and the various observations relating carbohydrate with uric acid metabolism,15uric acid clearance studies failed to uncover a defect of uric acid clearance in mild diabetics, utilizing a technic of uricosuric activity of glucose loads. Uric acid clearances and uric acid clearance/creatinine clearance ratios were found to increase in 5 diabetic subjects following administration of oral glucose loads. In 4 of the 5 subjects there was also a slight decrease in the serum uric acid levels. The diabetics studied exhibited a uricosuric acition of glucose comparable qualitatively to nondiabetics. This influence of glucose may explain the clinically observed improvement of gout with the onset of diabetes, but is not adequate to prevent the occasional coexistence of hyperuricemia with hyperglycemia.
