Abstract Human marrow cells were incubated with 59 Fe for 21 hours to produce 59 Fe-labeled erythroblasts. When these labeled cells were incubated with 6 pure red cell aplasia (PRCA) plasmas, 3 produced an increased release of 59 Fe from the erythroblasts into the supernatant media. The increased release of 59 Fe has previously been shown to represent a release of hemoglobin and is an index of erythroblast cytotoxicity. To characterize the effect of the PRCA plasmas a two-stage incubation method was adopted. Using this method 4 of 7 PRCA plasmas produced an increased release of 59 Fe. The plasmas of 10 of 11 polytransfused patients with diseases other than PRCA did not increase the release of 59 Fe above the normal level. Using the two-stage method it has been shown that marrow cell contact with PRCA γG-globulins is necessary in the first stage, and cell contact with fresh frozen normal plasma is necessary in the second stage. The second-stage normal plasma is inactivated by 56 °C. heat-treatment and by treatment with suramin, zymosan, and ethylene-glycol-tetra-acetic-acid (EGTA), 3 inhibitors of complement, indicating that the cofactor may be complement. The presence of the cytotoxic factor in the γG-globulin fraction and its apparent dependence on complement indicate that it may be an antibody or an immune complex.
