Low back pain is one of the most common reasons for seeing a physician in the United States. Of the 15 million office visits per year for low back pain, a large percentage are to primary care physicians. The generalist needs to be able to take a focused history and perform a directed physical /neurologic examination to select the small percentage of patients who require a more aggressive early diagnostic evaluation. Also necessary is a working knowledge of the major clinical syndromes. The generalist needs to know when to subject the patient to a diagnostic work-up and the approach to treatment often without need for referral to a specialist.
