Dear Editor, Rathke’s cleft cysts (RCCs) usually originate from the remnants of Rathke’s pouch arising from the primitive stomodeum. Based on the embryology of Rathke’s cleft, it is possible to explain the locations of RCCs, even the ones reported in very rare regions, such as entirely in the suprasellar region [4, 9], sphenoid sinus [6], and sphenoclivus [3]. On the other hand, the embryological theory cannot explain the origin of a RCC located in the anterior pituitary lobe. We herein report the first case of RCC located entirely in the anterior pituitary lobe, and hypothesize that the origin of this RCC might have been the migration of epithelial cells from Rathke’s cleft into the anterior wall of Rathke’s pouch due to the expression of leukemia inhibitory factor (LIF), which is one of the known endogenous factors.A 32-year-old female was referred to our department with a pituitary mass lesion detected by magnetic resonance (MR) imaging associated with headaches. A neurological examination on admission showed no abnormalities. Endocrinological examinations were within the normal ranges on admission. MR imaging demonstrated a nonenhanced intrasellar cyst appearing as an isointense region on T1-weighted (Fig. 1a) and T2-weighted images (Fig. 1b).
