A 33-day-old baby girl was brought with concerns regarding acholic stool colour. After laboratory examinations, abdominal ultrasound and hepatobiliary scintigraphy, biliary atresia was suspected, and the patient was transferred to our hospital for surgery. Biliary atresia was definitively diagnosed by cholangiogram, and the Kasai procedure was performed at 47 days of age, but her jaundice persisted. Her liver condition progressed to cirrhosis requiring liver transplantation. At 5 months of age, during pretransplant evaluation, physical examination revealed conjunctival icterus, hepatosplenomegaly and distended abdominal veins. Terry’s nails, characterised by a light pink to white nail-bed and an undetectable lunula with a narrow zone of pink to red at the distal end, were also observed on her fingers (figure 1).
