DEAR EDITOR, A 42-year-old immunocompetent woman presented with a 1-year history of a slowly growing mass on her left foot (a). Histopathology showed clumps of filamentous organisms and grains, the latter surrounded by matrix (b). Biopsy tissue culture grew Gram-positive Gordonia (also known as Gordona terrae) (c, d), which was confirmed by 16S rRNA sequencing. Magnetic resonance imaging demonstrated cellulitis without involving the bone (e). A diagnosis of actinomycetoma was made. The lesion was resolved after being treated with amoxicillin and clavulanic acid for 3 months (f). Mycetomas, commonly occurring on the foot, are chronic, progressively destructive, granulomatous infections caused by fungi (eumycetoma) or bacteria (actinomycetoma). G. terrae is ubiquitous in the environment and often isolated from soil and water. Actinomycetoma caused by G. terrae is very rarely reported. G. terrae can produce pale grains in cutaneous tissue. Gordonia, Nocardia, Rhodococcus and Mycobacterium can easily be misidentified on smear microscopy because they are all actinomycetes.
