The imaging community desperately needs an easy-to-perform and readily available imaging technique for evaluating chronic kidney disease (CKD). Chronic kidney disease affects a large proportion of the world population—greater than 800 million individuals [1]. Patients with CKD may present with edema or hypertension. Yet, many CKD patients are asymptomatic and are identified incidentally via elevated serum creatinine or derangements on urinalysis. CKD may also be detected incidentally on imaging when the kidneys have atrophied and/or are echogenic on ultrasound [2].Ultrasonography is the most common radiologic technique used in patients presenting with renal dysfunction given its availability and ease of use. Patients presenting with newly identified renal dysfunction—via elevated serum creatinine—may be acute or chronic. Ultrasound provides limited information indicative of chronicity; chronicity is suggested by renal atrophy. Therein, however, is the problem with the sonographic evaluation of CKD: the evaluation is rudimentary—providing essentially a binary answer of present/absent. The severity of CKD is not assessed. An imaging-based technique is much needed that can provide a quantitative assessment of CKD, as well as early detection of parenchymal changes.
