Chorea gravidarum, a bizarre movement disorder involving the limbs and face in pregnancy used to be a reasonably common condition, but is now rare. Willson & Preece (1932) in their review of 951 cases thought the incidence was about 1 :3500 pregnancies with a maternal mortality rate of 12.7%. A recent estimate by Zegart & Schwarz (1968) suggests an incidence of 1 : 139 000 pregnancies; a recent maternal mortality rate of about 1% has been suggested by Lewis & Parsons (1966). A single case of chorea gravidarum has been described in a patient with systemic lupus erythematosus (SLE) by Donaldson & Espiner (1971). We present a patient in whom chorea gravidarum was associated with a circulating lupus-type anticoagulant but not with SLE. This pregnancy was successfully completed under cover of immunosuppressive doses of prednisone and low-dose aspirin therapy. The extremely distressing movement disorder responded to tetrabenazine which had no apparent adverse effects on fetal growth and welfare.
