In their Comment (Dec 18, p 2160) 1 on our study to compare episiotomy with fetal manipulation for managing severe shoulder dystocia, 2 Andrew Breeze and Christoph Lees interpret the emphasis of our conclusions rather curiously. We write to correct their description of our methods and explicate our purpose in undertaking this research. By definition, our novel criteria for objectively selecting severe shoulder dystocia deliveries were applied only retrospectively. In the subsequent analysis, we separated the severe dystocia deliveries into three groups on the basis of the performance of episiotomy, fetal manipulation, or both. There was no relation—nor could there have been—between the clinicians’ choice of management technique and the particular criterion (often more than one) by which the shoulder dystocia was classified as severe.
