In this presentation, the effect of topical application of dexamethasone on intraocular pressure and its dynamics in hypertensive open-angle glaucoma and in low-tension glaucoma was reported. In comparable age groups, the dexamethasone-induced hypertension was always markedly greater in the glaucomatous than in the normal eye. It was of equal magnitude in low tension and in hypertensive open-angle glaucoma. The dexamethasone induced hypertension was responsive to hypotensive medical therapy. On the other hand, effective hypotensive therapy could not protect the glaucomatous eye from the dexamethasone effect. The dexamethasone effect in low tension glaucoma showed that the trabecular meshwork is altered in a manner similar to that of hypertensive open-angle glaucoma, revealing the intimate basic simi[long dash]larity of the 2 disease categories. The practical implications and the clinical significance of these findings, as well as the studies necessary for the complete understanding of the dexamethasone effect on intraocular pressure were discussed.
